Part of the reason Draco is so strongly affected is that he never really faced the feelings losing Harry caused in him. He just shoved him away as hard as he could and then pretended everything is beautiful and that ugliness won't hurt him if he doesn't think about it. That's particularly visible in Chapter 2.
