Well, his reaction is definitely exaggerated by the fact that he's in the Malfoy home, the center of their power, and doesn't feel he can trust them. But yes, some of it is that he's gotten used to neglecting his own health while he heals his patients, and it will take a lot of work to make him reverse that.
