Draco's reaction and feelings are actually very human, doesn't seem like a reaction of a shallow person at all, but rather someone dealing with a physical reaction they can't help, repulsion, knowing they probably should be a better person than to feel that way and trying to defend themselves emotionally the best way they can or know. I guess he just has to look through another angle to find a type of beauty that overcomes the ugliness he sees, because I doubt that someone is really able to have a healthy physical relationship with someone who repulses them, no matter how wonderful and worthy of love that person is.
And yes, Harry's need to be honest about himself is very legit, but Draco's need of being in a sexual relationship with someone who doesn't make him throw up is also legit (in fact I believe he's entitled to that, like everyone else), and if Draco wasn't being reasonable in wanting to simple hide Harry's condition, wanting to impose a change like that to your lover and expect him to accept it and ignore his own reactions to it isn't very reasonable either.
